Brief description of trial (Data source: BASEC)
Liegt das oben beschriebene Krankheitsbild vor, wird der Patient und die Sorgeberechtigten über die Studie aufgeklärt.
Mit Einwilligung erfolgt eine Randomisierung des Patienten in eine Studiengruppe. Randomisierung bedeutet, dass der Patient über einen Computer zufällig einer Studiengruppe und damit einer Studienmedikation zugeteilt wird. Entsprechend der Gruppenzuteilung erhält der Patient die Standardtherapie und ein zusätzliches Medikament (Immunglobuline oder Kortikosteroide). Sowohl Kortikosteroide wie auch Immunglobuline haben sich in der Vergangenheit als wirksam bei der Behandlung ähnlicher Krankheitsbilder gezeigt und werden auch bei PIMS-TS basierend auf Expertenempfehlungen eingesetzt. In jedem Fall und unabhängig von der Studiengruppen-Zuteilung erhält der Patient eine nach aktuellem Wissen verfügbare und wirksame Therapie.
Während des Spitalaufenthalts wird der Patient studienunabhängig überwacht. Hierzu zählen unter anderem auch klinisch notwendige Blutentnahmen oder bildgebende Verfahren. Über diese Massnahmen und deren Häufigkeit entscheidet der behandelnde Arzt. Es sind keine studienspezifischen Untersuchungen im Spitalaufenthalt geplant, allerdings können Informationen und Daten, die während des Spitalaufenthalt erhoben werden bei Bedarf für die Studie mitverwendet werden.
Nach Spitalentlassung erfolgen ambulante klinisch notwendige Nachkontrollen des Patienten, zu bestimmten Zeitpunkten im Spital. Auch erfolgen einige (ca. 2-3) studienspezifische Nachkontrollen. Diese können zum Teil telefonisch oder im Spital, idealerweise in Kombination mit den klinischen Kontrollen erfolgen. In den studienspezifischen Kontrollen werden z.B. spezifische Fragebögen (Lebensqualität, Rehabilitation, Impfungen) erhoben.

PIMS-TS (Paediatric Inflammatory Multisystem Syndrome - Temporally associated with SARS-CoV-2) ist ein mit dem Coronavirus vorkommendes erst kürzlich identifiziertes Krankheitsbild bei dem es zu einer körpereigene, überschiessende und unkontrollierte Entzündungsreaktion kommt. Diese kann verschiedene Organsysteme betreffen. PIMS-TS führt häufig zu schweren Krankheitsbildern, bei denen eine Betreuung auf der Intensivstation wegen Atem- und Kreislaufunterstützung notwendig sein kann.

- hospitalisierte Patienten < 18 Jahre
- Patienten mit SARS-CoV-2 Infektion (klinisch oder laborchemisch) mit Anhalt für PIMS-TS (Paediatric Inflammatory Multisystem Syndrome - Temporally associated with SARS-CoV-2)
- Keine bestehende Medikamentenanamnese, welche den Patienten bei Studienteilnahme einem möglichen zusätzlichen Risiko aussetzen könnte

- Neugeborene mit einem korrigierten Gestationsalter von <= 44 Wochen

Inclusion Criteria:
- Hospitalised children (aged <18 years old)
- SARS-CoV-2 infection associated disease (clinically suspected or laboratory confirmed)
with evidence of single or multi-organ dysfunction (called Pediatric Multisystem
Inflammatory Syndrome temporally associated with COVID-19 [PIMS-TS]).
- No medical history that might, in the opinion of the attending clinician, put the
patient at significant risk if he/she were to participate in the trial
Exclusion Criteria:
- Neonates/infants with a corrected gestational age of <= 44 weeks
- If the attending clinician believes that there is a specific contra-indication to one
of the active drug treatment arms or that the patient should definitely be receiving
one of the active drug treatment arms, then that arm will not be available for
randomisation for that patient.
