Klinische Prüfung der Phase III mit CPX-351 zur Behandlung der Myeloischen Leukämie bei Kindern mit Down Syndrom 2018

Eine myeloische Leukämie ist eine bei Kindern und Jugendlichen seltene Blutkrebserkrankung, die den ganzen Körper betrifft. Kinder mit Down-Syndrom haben ein stark erhöhtes Risiko eine myeloische Leukämie zu entwickeln. Obwohl sie eine bessere Prognose im Vergleich zu Kindern ohne Down-Syndrom haben, leiden diese Patientinnen und Patienten vermehrt unter den Nebenwirkungen der intensiven Chemotherapie. Aufgrund dieser Aspekte ist eine spezielle Behandlung für Kinder mit ML-DS notwendig. Ziel dieser Studie ist zu beurteilen, ob durch Einführung des neuen Medikaments mit dem Namen ‘Vyxeos’ die Rate an Nebenwirkungen gesenkt werden kann, ohne die Heilungschancen zu reduzieren. In der Studie wird untersucht, wie Kinder auf diese Therapie ansprechen und welche Nebenwirkungen sie zeigen. Gleichzeitig soll die Medikamentendosis gegen Ende der Behandlung für die Kinder, die gut auf die Chemotherapie angesprochen haben, verringert werden, um die Nebenwirkungen zur verringern. Aufgrund der guten Daten von vorangehenden internationalen Studien wird davon ausgegangen, dass diese Behandlung zu einer ähnlichen Überlebensrate führt, aber akute Nebenwirkungen und Spätfolgen der Therapie stark reduziert werden können.

Wie auch die Standardbehandlung, gliedert sich die Studienbehandlung in vier Zyklen intensiver Chemotherapie, die jeweils 28 Tage dauern. Im Gegensatz zur Standardbehandlung wird in der Studie in den ersten beiden Zyklen das Medikament Vyxeos verabreicht. Vyxeos ist bereits zur Behandlung von akuten myeloischen Leukämien bei Erwachsenen zugelassen und erzielt vielversprechende Ergebnisse. Eine Zulassung für Kinder hat das Medikament bisher noch nicht, wenn gleich es bereits im Rahmen von Studien bei Kindern mit Leukämie erfolgreich eingesetzt wurde. Dies ist aber die erste Studie, in der Vyxeos bei Kindern mit Down-Syndrom und ML oder MDS eingesetzt wird.
Der dritte Zyklus ist identisch zur Standardbehandlung und besteht aus den Medikamenten Cytarabin und Mitoxantron. Auch der vierte Zyklus besteht aus diesen zwei Medikamenten. Allerdings wird im Rahmen der Studie nach dem ersten Zyklus zur weiteren Therapieplanung zusätzlich die Anzahl der übriggebliebenen Leukämiezellen im Blut und Knochenmark gemessen. Diese Untersuchung dient zur Einschätzung der Wahrscheinlichkeit, dass die Krankheit nach der Behandlung zurückkommt. Für Kinder mit einer niedrigen Rückfallwahrscheinlichkeit wird im vierten Zyklus eine reduzierte Dosis von Cytarabin im Vergleich zur Standardtherapie gewählt.
Um die Wirksamkeit und Sicherheit der Studienbehandlung korrekt und in allen Details zu studieren, wird einerseits eine standardisierte Diagnostik in sogenannten Referenzlaboren durchgeführt, und andererseits werden von allen teilnehmenden Patientinnen und Patienten medizinische Daten erfasst, gespeichert und gemeinsam ausgewertet.

Inclusion criteria:
•Myeloid Leukemia (ML) or Myelodysplastic Syndrome (MDS), according to WHO
•Trisomy 21: Down syndrome or mosaic
•Age: > 6 months and = 4 years of age with/without GATA1 mutation OR > 4 years of age < 6 years of age with GATA1 mutation
•Morphology/Immunophenotyping: FAB M0, M6 or M7
•Lansky performance score at least equal to 50; or Karnofsky performance status at least equal to 50, whichever is applicable
•Understand and voluntarily provide written permission of parental/legal representative(s) to the ICF prior to conducting any study related assessments/procedures, also concerning data and tumor material transfer according to ICH/GCP and national/local regulations
•Able to adhere to the study visit schedule and other protocol requirements
• Acceptance that vaccination with live vaccines is not possible while participating in the trial

Exclusion criteria:
•Children with Transient Abnormal Myelopoiesis (TAM), according to WHO

•Cytogenetics: AML with recurrent genetic abnormalities (WHO 2016)

•Previous allogeneic bone marrow, stem cell or organ transplantation

•Evidence of invasive fungal infection or other severe systemic infection requiring treatment doses of systemic/parenteral therapy including known active viral infection with human immunodeficiency virus (HIV) or Hepatitis Type B and C

•Symptomatic cardiac disorders (CTCAE 4.0 Grade 3 or 4)

• Diagnosed Wilson’s Disease

•Major surgery within 21 days of the first dose.

•Any anti-cancer therapy (e.g., intensive chemotherapy, biologics or radiotherapy) for more than 14 days or within 4 weeks before start of therapy, except low-dose cytarabine for the treatment of TAM.

•Concomitant treatment with any other anticancer therapy except those specified in protocol during the study therapy

•Treated by any investigational agent in a clinical study within previous 4 weeks

•History of hypersensitivity to the investigational medicinal product or to any drug with similar chemical structure or to any excipient present in the pharmaceutical form of the investigational medicinal product

•Former Enrolment to this study

•The patient concerned has been committed to an institution by virtue of an order issued either by the judicial or the administrative authorities
